I came across this today in  a customers house. It was probably a loose connection. When a plug is pushed into a socket the clamp in the socket is pushed apart to allow the plug pin in. Sometimes the spring mechanism fails and a bad connection is made. This is the result.

Before plugging in any appliance ensure that the lead is in good condition. If there are any signs of abrasion or blackening do not use the appliance until you get the plug checked out.
